個人情報保護方針PRIVACY POLICY

株式会社大分大学先端医学研究所（以下、当社といいます）は、当社が保有する個人情報を、「個人情報の保護に関する法律」および「行政手続における特定の個人を識別するための番号の利用等に関する法律」（以下、番号法といいます）その他の関係法令等に基づき、以下の通り適正に取り扱います。

当社は、適正かつ公正な手段により個人情報を取得します。また、個人情報の取得にあたって、あらかじめ利用目的を通知または公表します。なお、要配慮個人情報の取得にあたっては、法令で認められている場合を除き、あらかじめ本人の同意を取得します。

当社は、あらかじめ通知または公表した目的の達成に必要な範囲内で個人情報を取り扱います。

当社は、原則としてご本人の同意を得た場合または法令で認められている場合を除き、要配慮個人情報および個人データを第三者に提供しません（特定個人情報等については、法令で認められている場合を除き、第三者に提供しません）。ただし、利用目的の達成に必要な範囲内で要配慮個人情報もしくは個人データまたは特定個人情報等の取扱いを第三者に委託する場合は、委託先を必要かつ適切な範囲で監督します。

当社は、個人データおよび特定個人情報等を正確かつ最新の内容に保つよう努めるとともに、個人データおよび特定個人情報等ならびに匿名加工情報の漏えい・滅失または毀損の防止、安全管理のために、組織的・人的・物理的および技術的な安全管理措置を講じます。

当社は、ご本人から保有個人データの開示・訂正または利用停止等を求められた場合、遅滞なく合理的な範囲で対応します。また、個人情報の取扱いについてご本人から苦情を受理した場合は、事実を調査した上で、誠意をもって対処するよう努めます。

当社は、利用目的を達成した特定個人情報等については、できる限り速やかに廃棄または削除します。
また、本方針で使用する「個人情報」「要配慮個人情報」「個人データ」「保有個人データ」および「匿名加工情報」の用語は、個人情報の保護に関する法律第２条に定める定義の通りとします。また、「特定個人情報等」の用語は、番号法第２条に定める「個人番号」および「特定個人情報」とします。

Epo-med Inc. (hereafter “Epo-med” or “the Company”) recognizes the importance of personal information and the need to observe laws and regulations on the protection of personal information with the philosophy of respecting human rights, and handles personal information using lawful and appropriate methods.

Epo-med collects personal information using lawful and appropriate methods and announces or notifies the purposes of use.

Epo-med handles personal information it collects within the confines of relevant specific purpose of use.
In case personal information is required for purposes other than the specified purposes of use, the Company will obtain the consent of the corresponding person.

Epo-med will not provide personal information to any third parties without obtaining the consent of the corresponding person.

Epo-med takes safe measures to appropriately protect personal information against unauthorized access, loss, destruction, falsification and/or divulgation of the personal information.

Epo-med will continuously improve the system and mechanism to control the protection of personal information.
